• Ages 18+
• BMI less than 30 kg/m2
• Must not have a history of or current TMJD
• Must not have history of dental or surgical procedures for lower facial shaping or masseter reduction
• 15 visits over 12 months
• Injection treatment

Hidradenitis Suppurativa

Must have tried ONE of the following medications in the past for the specific amount of time and did not have success:

  *For 12+ weeks: Infliximab-Remicade, Adalimumab-Humira, Etanercept-Enbrel

  OR

  *For 16+ weeks: Certolizumab-Cimzia, Secukinumab-Cosentyx, Ixekizumab-Taltz, Brodalumab-Siliq

• Ages 12 and older
• Up to 2.5 years duration
• Oral extended-release tablet
• Qualified patients may receive up to $1,820

Upcoming: Actinic Keratosis (body)

Must have AK’s on the main body area (e.g. back, shoulders, chest, arms, hands, legs)

• Ages 18 and older
• 6-8 visits over 1-1.5 years
• Topical gel + red light photodynamic therapy (PDT) – 1 or 2 treatments total
• Qualified patients may receive up to $760 – $1,070

Plaque Psoriasis

Must be a male or a female not of childbearing potential

• Ages 18 to 75
• Moderate to severe amount of psoriasis
• Plaques must be moderate to severe in severity
• 13 visits over 7 months
• Oral capsule treatment

Must have Plaque Psoriasis covering 10%+ of body (excluding scalp)

• Ages 18+
• Moderate to severe amount of psoriasis
• Plaques must be moderate to severe in severity
• 23 visits over 156 weeks
• Topical and Photoherapy treatment
• Qualified patients may receive compensation

Must have at least 3 but not more than 20 warts!

• Ages 12+
• Must not have a history of keloid formation
• 14 visits over 30 weeks
• Injection treatment

Must have non-segment vitiligo on both the face and main body areas

• Ages 18 and older
• 11 visits over 1 year
• Oral capsule treatment

Must have 1 – 8 common warts (usually found on the hands or fingers)

• Ages 12-17
• Up to 14 visits over 6 months
